X-Series Advanced Cardiac Monitor Failures

In May 2023, Toronto Paramedic Services began deployment of new Zoll X-Series Advanced Cardiac Monitors to replace X-Series Cardiac Monitors. Since deployment, three (3) X-Series Advanced Cardiac Monitors have failed to turn on when required. Fortunately, there have been no adverse outcomes.

Zoll Canada has determined that the failure of these three (3) X-Series Advanced Cardiac Monitors was due to a problem with the electrostatic discharge protection mechanism which failed to reset after engaging to protect the monitors. Zoll Canada and Toronto Paramedic Services are reviewing all known causes for electrostatic discharges.

  • As a precaution, all X-Series Advanced Cardiac Monitors will be returned in batches for rigorous environmental testing.
  • Toronto Paramedic Services has ceased further deployment of X-Series Advanced Cardiac Monitors into Operations until such testing has been completed.

In the event an X-Series Advanced Cardiac Monitor fails to turn on and there is sufficient power supply (battery or AC power), on-scene troubleshooting is unlikely to resolve this issue. Paramedics will:

  • Prioritize transport, where appropriate, or use cardiac monitors available on-scene from Allied Agencies or request an additional unit.
  • Notify their Superintendent as soon as possible to get a replacement cardiac monitor.
  • Document in the ePCR that the cardiac monitor was not functioning, any treatments that were not performed because of the failure and complete an Incident Report.
