Humber River Hospital EMS Triage Corridor Closure – May 7-14, 2024

The main emergency hallway at Humber River Hospital will be under construction from May 7-14, 2024. This construction requires the closure of the EMS entrance from the ambulance bay, the corridor Paramedics currently use for offload delay and access to O Zone.

During this time, Paramedics should continue to park in the garage and enter the hospital through the decontamination room. Paramedics are to follow EMS Triage signage (Figure 1) to the temporary triage station located at the intersection of O Zone and Acute (Figure 2).

Updated Call Response Changes Stage 3 – Referrals to Toronto Fire Services

Temporary response model changes implemented to respond to the COVID-19 Omicron variant continue to be scaled back as Toronto Paramedic Services reverts to our pre-COVID response plan.

CACC Deployment to Back-up Ambulance Communications Centre – March 13-14, 2024

On Wednesday, March 13 all CACC Operations will deploy to the Back-up Ambulance Communications Centre at 703 Don Mills Rd. (8th floor). The planned return to the primary Communications Centre at 4330 Dufferin St. is for the Night Shift on Thursday, March 14. This deployment is required to complete maintenance, and finalize construction required as part of the CACC expansion. There is no server switch (i.e., Non-CAD Environment) associated with this transfer of operations.

IMPORTANT NOTE: Staff working Wednesday, March 13, and Thursday, March 14, C-SHIFT ONLY (including Senior EMDs at 10:00, and EMDs and Call Takers at 11:00 and 14:00 start times) will report to 4330 Dufferin St and work out of the EDQI Classroom. Additionally, any Call Taker Training (including Days at 07:00) will take place in the EDQI Classroom at 4330 Dufferin St.

Procedure – Deployment to 703 Don Mills Rd.

The deployment to 703 Don Mills Rd. will take place during an early shift change on Wednesday, March 13.

  • Staff from the Tuesday NIGHT shift will remain at 4330 Dufferin St.;
  • Incoming Wednesday DAY shift staff will report to 703 Don Mills Rd.;
  • Wednesday DAY shift Senior EMDs will start at 05:30 (0.5 hours of overtime will be paid) and the Tuesday NIGHT shift Senior EMDs may be asked to remain until 06:30 (if needed) to ensure a smooth transition;
  • Wednesday DAY shift EMDs and Call Takers will start at 06:30 (0.5 hours of overtime will be paid) and Tuesday NIGHT shift EMDs may be asked to remain until 07:30 to ensure a smooth transition; and
  • Wednesday DAY shift Superintendents and Deputy Commanders will start at 05:30 (0.5 hours of overtime will be paid) and Tuesday NIGHT shift Superintendents and Deputy Commanders may be asked to remain until 06:30 to ensure a smooth transition.
  • Please see the note above regarding staff on c-shift (Senior EMDs, EMDs and Call Takers) as well as staff currently in Call Taker training.

Additional CACC staff (including EDQI and CACC IT) will be on duty to assist with the transition.

Procedure – Return to 4330 Dufferin St.

The return to 4330 Dufferin St. will take place during an early shift change on Thursday, March 14.

  • Staff from the Thursday DAY shift staff will remain at 703 Don Mills Rd.;
  • Incoming Thursday NIGHT shift staff will report to 4330 Dufferin St.;
  • Thursday NIGHT shift Senior EMDs will start at 17:30 (0.5 hours of overtime will be paid) and the Thursday DAY shift Senior EMDs may be asked to remain until 18:30 (if needed) to ensure a smooth transition;
  • Thursday NIGHT shift EMDs and Call Takers will start at 18:30 (0.5 hours of overtime will be paid) and Thursday DAY shift EMDs and Call Takers may be asked to remain until 19:30 to ensure a smooth transition.
  • Thursday NIGHT shift Superintendents and Deputy Commanders will start at 17:30 (0.5 hours of overtime will be paid) and Thursday DAY shift Superintendents and Deputy Commanders may be asked to remain until 18:30 to ensure a smooth transition.

Additional CACC staff (including EDQI and CACC IT) will be on duty to assist with the transition.

Headsets

All staff who will be working at 703 Don Mills Rd. at any time during the affected weekend must bring their personal headsets (all pieces) with them and must bring their headsets back to 4330 Dufferin St. for their next scheduled shift.

Staff Parking at 703 Don Mills Rd.

Between Wednesday, March 13 and Thursday, March 14, parking for personal vehicles will be provided on the upper deck of the 703 Don Mills Rd. parking structure. All spaces must be vacated by 19:30 on Thursday, March 14.

Staff requiring accessible parking may use the designated spots outside the main entrance to 703 Don Mills Rd. with a valid permit visible.

Auto-Functions in InformCAD

All Auto-Roster, Auto-Meal Break, and Auto-Status changes will be operational while staff are deployed to 703 Don Mills Rd.; however, EMDs must be vigilant regarding status updates, meal breaks, and process changes, and be prepared to intervene using InformCAD or radio communication with Paramedics as needed.

Patient Distribution System – Bypass Agreements and Specialty Services

The Patient Distribution System (PDS) includes a list of specialty services and bypass agreements to help ensure patients are transported to the most appropriate hospital for their medical emergency. In collaboration with our hospital partners, the list and definition of the specialty services included in PDS are being updated to accurately reflect the care that hospitals can provide.

Effective March 5, 2024, the following bypass agreements and specialty services will be included in PDS:

  • Paediatric (<18 years of age)
  • Field Trauma Triage Guidelines (FTT)
    • Adult ≥16 years of age
    • Paediatric < 16 years of age
  • Ortho (ONLY for a suspected shoulder or hip fracture)
  • Psychiatric
    • Adult ≥18 years of age
    • Pediatric <18 years of age
  • Code Stroke
  • Code STEMI
  • Stroke Non-protocol (acute stroke symptoms outside the Code Stroke bypass guidelines)
  • Burn
  • Dialysis (for any patient receiving dialysis treatment, regardless of the emergency)
  • Isolated Appendage Amputation
  • Obstetrics (ONLY for patients ≥20 weeks pregnant; patients are to be taken directly to the obstetrics floor)
  • Psych Peds Form X (Formed Paediatric Psychiatric) (<18 years of age)
  • Stabilization
  • Transplant 

There is no change to the process for EMDs. The EMD should continue to select the appropriate CTAS and service(s) requested through the main buttons or dropdown menu.  A hospital resource guide is available on the G drive: EMD information.

As a reminder, per PDS rules:

CTAS 1 patients will be directed to the closest hospital, regardless of the specialty service provided. For Code Stroke/STEMI and FTT transports, Paramedics will be permitted to transport to the hospital that offers the service unless the patient meets bail-out conditions. For CTAS 1 paediatric and obstetric services, there are a select few hospitals in the core that will remain excluded based on proximity to the hospital with the specialty service.

CTAS 2A-5 patients will only be assigned a hospital where the service is provided.

Ontario Health, hospital leadership, and our emergency department (ED) partners are aware of the updates, and changes will be reviewed regularly with all hospital partners. If you have any questions or concerns, please speak to your Superintendent.

Implementation of Six Sector Dispatch Model

On March 20, 2024, Toronto Paramedic Services will be implementing a six sector dispatch model, fully aligned with Operations. This dispatch model is designed to better balance unit distribution among sectors and improve sector EMD workload, situational awareness and communication between EMDs and Paramedics.

The Six Sector Dispatch Model will go live on March 20, 2024, at 06:00.

This change involves the creation of two new dispatch sectors and radio channels – North Central (D5) and South Central (D6) – as well as a redistribution of stations to fully align the new dispatch sectors and Operations district boundaries. D5 will now provide geographic coverage in addition to existing Special Operations responsibilities.

Additional information will be distributed in the coming weeks and staff will have the opportunity to participate in a practical refresher training session.

Updated Call Response Changes – Referrals to Toronto Fire Services

Effective today, February 12, 2024, Toronto Fire Services (TFS) staff are unable to clear from calls with an identified patient prior to Paramedic arrival. Since firefighters do not have an ‘aid to capacity’ assessment to confirm whether a patient understands the risks associated with a refusal of care, Paramedics are required on scene for all calls where there is a confirmed patient before TFS may clear.

The second stage of the rollback to the pre-COVID response plan has been adjusted to align with this change. TPS has now resumed primary response for the following determinants:

In anticipation of this change, a review of all TFS-Referral calls was completed. The following changes to call responses are effective February 12, 2024:

  • TFS will only be tiered to calls on Protocol 17 (Falls) where the suffix ‘A’ for access issues is applied.
  • The Courtesy Code 2 Priority will return for Public Assist (Lift Assist) and Transportation Only Determinants.

These changes will be applied to CAD and ProQA; no special action is required from EMDs or Call Takers (CTs) during the processing of the original call. There are no changes to referrals to Telehealth/Health811.

When TFS is on scene for a TFS-Referral and request Paramedics attend, the EMD or CT is required to reprocess the call through ProQA using updated information from TFS. If reprocessing the call does not result in an upgraded priority, the EMD or CT will manually upgrade the call to a minimum ALPHA response. The same would apply if Police were on scene requesting

When TFS/Police are on scene and requesting Paramedics via a call-back, if the priority of the call remains an ALPHA, the controlling dispatcher, in consultation with One Desk, shall consider the use of the ‘No Divert (expt D/E)’ Out of Service (OOS) code to ensure the timely arrival of a Paramedic crew.

Lastly, at the discretion of One Desk, TFS may still be used as first response during call deferral. If a patient is identified, TFS will not be able to leave the scene until Paramedics arrive. Therefore, if TFS is asked to attend an ALPHA or lower priority call during call deferral, EMDs should dispatch Paramedics with the ‘No Divert (expt D/E)’ OOS code as soon as an appropriate unit becomes available.

Communications – Operations Ride Out Program

Beginning in spring 2024, Communications staff will have the opportunity to ride out with a Paramedic crew for a 12-hour observation shift. This opportunity will support greater awareness of operational procedures and demands, as well as relationship building between Communications staff and Paramedics.

Observation shifts are voluntary and are available to all active Communications staff. Interested staff will be assigned in seniority order and as permitted by operational demand. Staff will be notified in advance and are expected to book on at their assigned station. Shifts will be scheduled on day or c-shift start times, and if end-of-shift overtime occurs, transportation arrangements will be made to return Communications staff to the book-on station.

Prior to the observation shift being scheduled, staff must express interest via this survey, in which requests can be made for a preferred district, crew, and transport vs. response unit. Staff are required to complete a brief online orientation program and participate in PPE training prior to the scheduled shift. Ride-out kits, which include safety equipment and PPE, must be picked up prior to the observation shift and returned as soon as possible following the shift. The kits will be shared with all users of the program; therefore, a timely return is important to ensure everyone is equipped for their shift.

Booking Remaining 2023 Vacation Allotment for Full-Time Staff

With the 2024 vacation booking process now complete, we will begin booking any unused 2023 vacation and floating holidays.

Starting at 07:00 on Tuesday, February 20, 2024, full-time Communications Centre staff (EMDs, Senior EMDs, and Full-Time Call Takers) will be contacted in order of seniority to book their unused 2023 vacation and floating holidays. Only staff with 2023 carryover days will be contacted.

Vacation and floating holidays will be granted based on availability and seniority.

Staff who will not be available to be contacted must submit their vacation and/or floating holiday requests by email to Superintendent Kelly Clarke, by Wednesday, February 14, 2024, at 15:00, with the following:

  • Subject Line: Remaining 2023 Vacation Allotment
  • Requested dates in order of preference (this list will be used to book available dates, in order of seniority).

The booking process and rules will follow those outlined in the memorandum titled 2024 Vacation Booking for Communications Centre Staff released on November 23, 2023.

Upon the completion of this process, any requests for vacation, floating holidays, or lieu time within a current cycle must be submitted to your SCS for consideration. Any requests outside the current cycle will continue to be submitted to the Administrative Superintendent and/or designate for consideration.

Call Response Changes – Referrals to Toronto Fire Services

As a result of system pressures caused by the COVID-19 Omicron variant, Toronto Paramedic Services (TPS) and Toronto Fire Services (TFS) implemented temporary response model changes to protect ambulance availability for critical and life-threatening calls, and to ensure a timely response for lower priority calls.

These changes included referring a subset of calls to TFS where there is no clear indication of a patient or injury identified during the call-taking process or where the call was deemed very low acuity.

Currently, TPS continues to refer TFS to five (5) call types (36 call determinants), equating to approximately 55 requests for service per day and an average of nine (9) transports per 24 hours citywide.

TPS will once again become the primary responders for the low-acuity call types that were temporarily assigned to TFS during the pandemic, reverting to our pre-COVID response plan. These changes will be introduced gradually over a period of several months.

The first stage of the rollback to the pre-COVID response plan will take effect on January 31, 2024, when TPS will again become the primary responder for the following determinants:

Protocol 4 equates to an average of 21 requests for service per day and results in an average of five (5) transports per day. TPS attended to 55% of all Protocol 4 calls that were referred to TFS and transported patients on approximately 30% of these calls.

Please continue to ensure that scene safety information as well as updates from the scene are recorded in the Comments/Notes tab and verbally communicated to TFS in line with current SOPs.

Updated Scene Safety Assessment (SSA) Procedures for EMDs

Effective Monday, January 29, 2024, the procedure for documenting a Paramedic Scene Safety Assessment is being updated with a new Powerline command, OOS Reason Code and timeline. This update will replace the current procedure laid out in the Dispatch Training Manual and the requirement to manually enter comments into the ticket. 

When a Paramedic crew is at scene and declares their intent to begin a Scene Safety Assessment (SSA), EMDs will now use the new Powerline command “SSA.”

Typing SSA <unit #> into Powerline will automatically execute the following tasks:

  • SA out of service reason code applied to the unit 
  • “Started Scene Safety Assessment” saved as a comment into the incident
  • Advisor Notification sent to Deputy Commanders 

EMDs are still required to notify the SEMD, the applicable DOS, and have the Paramedic crew switch to B9 (or other designated) radio channel.

 If an SSA exceeds 15 minutes, the SEMD will update the status of the crew to “Staged” and notify the EMD.

Re-opening of Trillium Urgent Care Centre - Communications

Starting Monday, January 29, Trillium Health Partners (THP) will re-open the Urgent Care Centre (UCC) located at 150 Sherway Drive. The UCC will be open to support the community between 15:00-21:00 hours, seven days a week.

The UCC will operate in a modified capacity, targeting lower acuity conditions, aligning closely to services provided by walk-in clinics. Patients presenting to the UCC with higher acuity conditions that require emergency care will be sent for management at Trillium Mississauga Emergency Department (ED).

When processing a call for the UCC, choose the CAD Premise:

“ALC Queensway Health – ALL UNITS”

As there are multiple clinics located at 150 Sherway Drive, Communications staff need to be sure to include secondary location information indicating whether the patient is in the UCC or another location, as applicable.

All calls from the Trillium UCC will be processed using the Emergency Call Taking form and Protocol 33. All policies and procedures regarding the use of Protocol 33 apply. The reason Protocol 33 is used is due to the expansion of Protocol 33 to now include all ALC, CH, NH, and RH premises.

Deputy Commander approval for transport from 150 Sherway Drive to the Mississauga Trillium ED is not required. The Patient Distribution System is configured to present MISS as the destination for all CTAS levels. Transport to an alternate local hospital is also permitted if the request is made, or if there is a valid repatriation reason.

THP is engaging the community to educate residents on the types of conditions appropriate for the UCC. The UCC will be able to support acute care for more complex conditions that present to their clinic and have put processes in place to transfer patients via non-emergency transfer services if continued care is required in a hospital ED. Paramedics may be utilized to facilitate transport of critically ill patients from the UCC to Mississauga Trillium ED.

Installation of OPTIMA Version 23.3.1 and Updated Logic

On Wednesday, January 24, 2024, the newest version of OPTIMA, Version 23.1, will be installed in the Communications Centre.

The installation process will begin at 05:00 and will take approximately one hour to complete. OPTIMA will be unavailable during this time. EMDs will have full use of CAD for all call taking and dispatch functions, and EDQI and IT staff will be on site during the installation period.

The new version of OPTIMA:

  • Looks and functions the same as the current version;
  • Interacts with CAD in the same way as the current version;
  • Includes a new “Look for Closer” feature;
  • Includes new and updated dispatch logic recommendations.

Fit2Sit - Destination Changes

Since Fit2Sit was implemented one year ago, Paramedics have enrolled more than 4000 patients, and the continuous quality assurance review has demonstrated the program’s safety when the inclusion and exclusion criteria are followed.

Based on feedback from Paramedics and with support from our hospital partners, qualifying Fit2Sit patients will now have greater flexibility in their hospital destination choice.

The Hospital Destination Coordinator (HDC) will follow the current process for destinating a Fit2Sit patient; however, effective December 21, 2023, PDS will now provide up to three hospital options.

The HDC shall provide the Paramedic crew with the first hospital option displayed in PDS. If the Paramedic requests a specific hospital and it is one of the three options displayed, the HDC may select this hospital destination.

Additional PDS Fit2Sit criteria:

  • If the transport time to a hospital is greater than 16 minutes, it will be automatically excluded from the PDS hospital recommendations.
  • Fit2Sit will only be allowed by PDS for patients who are CTAS 3-5.
  • PDS will assign 15 minutes of ‘Time to Next Patient’ (TNP) credit for all Fit2Sit Patients.

Paramedics may use the term “ambulatory” when requesting a hospital destination. However, this term has no impact on the hospital options shown by PDS and there is no action for the HDC to take when it is included in the destination request. As a reminder, the ‘Fit2Sit’ option in PDS is for Fit2Sit patients only and should not be used for an ambulatory patient. Offloading the Fit2Sit patient to the waiting room is at the discretion of the Paramedic and not the hospital.

ProQA 5.1.1.48 Installation

On December 21, 2023, at 06:00, ProQA will be upgraded to version 5.1.1.48. The update process will take approximately 15 minutes and ProQA can still be used to process calls while the update is in progress.

During the update, Call Takers must manually enter the Nature/Problem field on the Emergency Call Taking form if ProQA does not send the determinant to CAD. Once the new version of ProQA is installed and re-launched, it will resume normal operation.

This update will include changes to Case Entry, Chief Complaint Selection Rules, Protocols 17, 23, 25, and Case Exit.

A complete description of the changes are available in the 5.1.1.48 Release Notes from the International Academy of Emergency Dispatch. A Quick Reference Guide summarizing the most pertinent changes is also available. See the following links:

There will be a six-week grace period for any deviations related to these changes for call reviews, with feedback only noted in Incident Performance Reports. Effective January 31, deviations associated with these changes will be applied.

Staff Parking at 703 Don Mills Rd.

Between Friday, December 15 and Tuesday, December 19, parking for personal vehicles will be provided on the upper deck of the 703 Don Mills Rd. parking structure. All spaces must be vacated by 07:30 on Tuesday, December 19.

Staff requiring accessible parking may use the designated spots outside the main entrance to 703 Don Mills Rd. with a valid permit visible.

Auto-Functions in InformCAD

All Auto-Roster, Auto-Meal Break, and Auto-Status changes will be operational while staff are deployed to 703 Don Mills Rd.; however, EMDs must be vigilant regarding status updates, meal breaks, and process changes, and be prepared to intervene using InformCAD or radio communication with Paramedics as needed.

2024 Vacation Booking - December 11-13, 2023

The 2024 vacation booking process for all Communications Centre staff (EMDs, Senior EMDs, and Full-Time Call Takers) will occur on December 11-13, 2023, from 07:00-17:00 daily.

Any staff not scheduled to be at work December 11-13, 2023, must ensure their contact information is up to date and that they can be reached at the phone number on file. Staff can update their ‘Contact Methods’ by accessing Kronos and navigating to the ‘My Info’ page.

Staff who will be unavailable during the vacation booking window must submit their vacation requests via CheckMarket survey. Staff should include multiple vacation dates, in order of preference, to ensure their vacation allotment is booked (the Scheduler will use this list to book available dates, in order of seniority). Staff who submit electronically will not be contacted by phone during the booking period. The vacation booking team will use the submitted date requests to book the staff member’s 2024 vacation allotment.

  • Staff will be contacted by the Communications Centre Scheduler in order of seniority, whether ON or OFF duty.
  • If a staff member misses the initial call from Scheduling:
    • A message will be left for the employee to call back as soon as possible at the phone number below.
    • The CACC Scheduler will wait 20 minutes and then attempt to call the employee again, leaving another message to call back as soon as possible at the phone number below.
    • The CACC Scheduler will wait another 20 minutes and attempt to call the employee for the final time. If the employee cannot be reached at that time, a message will be left stating the vacation booking process will continue with the next employee in seniority order.
    • If/when the employee calls back, their vacation request will be booked at the level of seniority being processed at the time of the callback.

Callbacks to the Communications Centre Scheduler
should be made to 416-338-7783.

  • Communications Centre staff are encouraged to book their entire annual vacation allotment during the initial vacation booking window. Requests outside of this window will be subject to availability.

Shift Blocks

  • Vacation must be booked in shift ‘blocks’. A block is defined as one continuous set of shifts. For example:
    • 4 Days = one block
    • 4 Nights = one block
    • 3 Days = one block
    • 3 Nights = one block
    • 2 Days = one block
    • 2 Nights = one block
  • The ‘make-up’ shift can also be combined with the set of shifts before or after to achieve a block. For example:
    • 3 days + Saturday (‘make up’) or Sunday (‘make up’) = one block
    • Saturday (‘make up’) or Sunday (‘make up’) + 3 nights = one block

NOTE: C-shift for the straight day shift rotation is considered the two days (starting on Thursday) and the following three days (starting on Monday).

Vacation Availability – Prime Time

Prime time for 2024 vacation is defined as May 16, 2024, to September 4, 2024, and December 16, 2024, to January 3, 2025, inclusive. The vacation allotments during prime time will be:

Floating Holidays (FH)

FH days may be booked along with vacation time for a full block of shifts.

Booking Single/Individual Vacation Shifts

Booking vacation for individual shifts will only be available during the non-prime time vacation period and will be granted based on seniority. The booking of one (1) individual vacation shift will be permitted once within each 24-hour period, over and above the numbers outlined in the Non-Prime Timetable above.

Carryover of 2023 Vacation

Staff interested in carrying forward their remaining 2023 vacation allotment into 2024 must submit their requests in writing directly to Brayden Hamilton-Smith, A/Commander, CACC, by December 3, 2023, at 23:59.

Approved 2023 vacation carryover will be booked after all full-time Communications Centre staff have been provided an opportunity to book their 2024 vacation. Vacation that has been carried forward will be booked subject to remaining availability and in order of seniority.

Cancelling Vacation

Booked vacation will not be cancelled, except in extenuating circumstances. Any requests for cancellation or postponement of booked vacation must be submitted, with the reason in writing, to the Administrative Superintendent and are subject to Commander review and approval. Requests will be evaluated on a case-by-case basis. Previously booked vacation dates that have been approved for cancellation will become available and may be rebooked using the Vacation Requests Made After the Booking Process detailed below.

NOTE: Staff are strongly encouraged to review the dates for any future special events of interest prior to booking their vacation. 2024 Special Event dates are tentative, subject to change, and are as follows:

  • Pride Toronto: June 28-July 1, 2024
  • Toronto Indy: July 19-21, 2024
  • Caribbean Carnival: August 3, 2024
  • Taste of the Danforth: August 9-11, 2024
  • CNE: August 16-September 2, 2024

Rebooking Vacation

If an employee needs to rebook any 2024 vacation (e.g., due to hospitalization, WSIB, training, leaves of absence, schedule changes, etc.), the employee must contact the Administrative Superintendent to request their vacation be rebooked. If the request to rebook cannot be accommodated, the employee will be advised of available shifts.

NOTE: If an employee chooses to bid on/switch to a new colour code and/or schedule, there is no guarantee that previously booked vacation will be honoured.

Vacation Requests Made After the Booking Process

Any request for vacation, FH, lieu time (LT), or statutory holiday time off outside of the annual vacation booking window must be submitted in writing for approval. Requests for time off prior to the cycle must be submitted to the Administrative Superintendent. Requests for time off during the cycle must be submitted through your assigned Superintendent. Approval of these dates will be dependent on availability and operational requirements.

A CUPE Local 79 representative will be present to observe and assist with the vacation booking process. Every effort will be made to process as many vacation bookings as possible each day. Staff are expected to be prepared to book their vacation at the time of the call to keep the process moving and to ensure all staff are provided an opportunity to book their vacation prior to the end of the day on December 13, 2023.

Superintendents and Deputy Commander Booking

Superintendent and Deputy Commander vacation booking will take place during separate booking sessions, consistent with previous years.

CPR Instructions – Return to Pre-Pandemic State

This memo replaces the direction provided in a previous memorandum released on May 5, 2020, titled CPR Instructions during the COVID-19 Pandemic”.

Effective immediately, all CPR instructions shall be given according to the appropriate pathway as laid out by the Medical Priority Dispatch System and ProQA. This is a return to the instructions in place prior to the COVID-19 pandemic.

Compressions-only CPR was implemented during the COVID-19 pandemic to help minimize the risk of exposure and spread of communicable diseases. While the risks associated with the spread of communicable diseases is ever-present, that risk has returned to a level that it is now appropriate to return to providing ventilation first instructions.

Callers have the right to refuse mouth-to-mouth instructions, and in such cases, the compressions-only pathway should be followed.

As a reminder, the majority of cases are automatically built into ProQA with the appropriate pathway. If prompted by the pathway director panel, choose the appropriate pathway as dictated by the circumstances of the call, per the graphic below:

Determinant Based Response

In spring 2024, Toronto Paramedic Services (TPS) will implement Determinant Based Response (DBR) to further refine our evidence-based approach of triaging emergency calls.

Using Medical Priority Dispatch System (MPDS) determinants, DBR has been developed through analysis of over three years of ePCR data captured by Paramedics, including interventions, vital signs, CTAS level, and the need for specialized transport (i.e., stroke, STEMI, etc.).

DBR is designed to provide EMDs with increased flexibility to select the most appropriate, Paramedic resource that aligns with patient needs. As a result, we anticipate more targeted and improved assignment of emergency calls.

More specific information about this initiative will be communicated in the coming months.
