University Health Network Reactivation Care Centre (CACC)

On Monday, June 17, 2024, the University Health Network (UHN) will open a new Reactivation Care Centre (RCC), UHN RCC (formerly known as Westpark), located at 82 Buttonwood Avenue.

The following map has been added to Geo as a bitmap in the CAD system:

The UHN RCC will support Alternate Level of Care (ALC) patients from Toronto General Hospital, Toronto Western Hospital, Mount Sinai Hospital, and Mississauga Trillium Hospital. Approximately 30 UHN patients are scheduled to be transferred to the new site throughout the week of June 17, with the remaining patients being transferred to the site through July and August.

As with the other RCC sites, when patients require transport back to their affiliated hospitals for non-acute reasons, they will be sent via non-emergency providers; however, in the event of an acute emergency, Paramedics will be dispatched on an appropriate response using Protocol 33.

Hospital Destination Coordinator

When a Paramedic requests a repatriation under extensive history for an RCC patient and the affiliated hospital request is within the City of Toronto boundaries, the HDC/EMD will:

  1. Select the appropriate CTAS level.
  2. Select the requested hospital destination, including the override reason of “Repat Extensive Hx” from the drop-down menu.
    1. When selecting an override, PDS will request an explanation in the comment field. Please document “RCC patient” in this section.

Notes

  • Deputy Commander approval is not required for the above repatriation                            
  • Deputy Commander approval is required for requests for Mississauga Trillium
