University Health Network Reactivation Care Centre (Operations)

On Monday, June 17, 2024, the University Health Network (UHN) will open a new Reactivation Care Centre (RCC), UHN RCC (formerly known as Westpark), located at 82 Buttonwood Avenue.

The UHN RCC will support Alternate Level of Care (ALC) patients from Toronto General Hospital, Toronto Western Hospital, Mount Sinai Hospital, and Mississauga Trillium Hospital. Approximately 30 UHN patients are scheduled to be transferred to the new site throughout the week of June 17, with the remaining patients being transferred to the site through July and August.

As with the other RCC sites, when patients require transport back to their affiliated hospitals for non-acute reasons, they will be sent via non-emergency providers; however, in the event of an acute emergency, Paramedics will be dispatched on an appropriate response.

Hospital Destination Requests 

The RCC will provide Paramedics with a patient transfer form indicating the patient’s medical history, medications, reason for transport and home hospital considerations. For CTAS 2B-5 patients, Paramedics should request a repatriation under extensive history for the RCC affiliated hospital.
