North York General Finch Reactivation Care Centre – Communications

On Saturday, December 17,, 2022, North York General Hospital will be opening their new Reactivation Care Centre (RCC), North York General Hospital Finch Site (formerly known as North York General Hospital – Branson Site), located at 555 Finch Avenue West.

The North York General (NYG) Finch RCC will support Alternate Level of Care (ALC) patients from North York General Hospital as they safely transition to the appropriate level of care.  Approximately 60 patients are scheduled to be transferred to the new site over the weekend of December 17-18, including the NYG patients currently residing at the Humber River Hospital Finch RCC site (ALC HRF).

As with the other RCC sites, when patients require transport back to their affiliated hospitals for non-acute reasons, they are generally sent via non-emergency providers, however in the event of an acute emergency, Paramedics will be dispatched on an appropriate response using Protocol 33

All previous ALC premises at 555 Finch Avenue West have been deleted.

Hospital Destination Coordinator

When a Paramedic requests a repatriation under extensive history for an RCC patient and the affiliated hospital request is within the City of Toronto boundaries, the HDC/EMD will:

  1. Select the appropriate CTAS level.
  2. Select the requested hospital destination including the override reason of “Repat Extensive Hx” from the drop-down menu.
    1. When selecting an override, PDS will request an explanation in the comment field. Please document “RCC patient” in this section.
    2. Deputy Commander approval is not required for the above repatriation.

At this time, the NYG Finch RCC will only house ALC patients that are associated with North York General Hospital.  As other sites come on board, this information will be communicated.
