North York General Finch Reactivation Care Centre – Operations

On Saturday, December 17, North York General Hospital will be opening their new Reactivation Care Centre (RCC), North York General Hospital Finch Site (formerly known as North York General Hospital – Branson Site), located at 555 Finch Avenue West.

The North York General (NYG) Finch RCC will support Alternate Level of Care (ALC) patients from North York General Hospital as they safely transition to the appropriate level of care.  Approximately 60 patients are scheduled to transition to the new site over the weekend, including the NYG patients currently residing at the Humber River Hospital Finch RCC site.

There will be no change to Paramedic response to 911 calls at this facility.  Paramedics will access the RCC via the entrance on the east side of the building (see map below).

As with other RCC sites, patients are considered to be admitted patients of their affiliated home hospital.  When patient acuity permits, per the Canadian Triage & Acuity Scale, all efforts should be made to repatriate patients to their affiliated hospital if it is within the City of Toronto boundaries.

When requesting a destination, Paramedics should request a repatriation under extensive history for an RCC patient.  Paramedics should be assigned the requested hospital without requiring an override from the Deputy Commander.

At this time, the NYG Finch RCC will only house ALC patients that are associated with North York General Hospital.  As other sites come on board, this information will be communicated.
