Mandatory COVID-19 Vaccinations for All Staff: Update #2

As you know, the City’s Mandatory Vaccination Policy requires all staff to disclose their vaccination status. In order to allow additional time for staff who do not have regular access to computers, the City has extended the deadline for the mandatory disclosure of vaccination status to Friday, September 17, 2021.

This extension does not change any of the other key dates in the Mandatory Vaccination Policy. Staff who are not fully vaccinated or who do not disclose their vaccination status by the deadline above will be required to complete mandatory training on the benefits of vaccination. Following the completion of the education session, employees will be required to receive one dose of COVID-19 vaccine by September 30, 2021, and two doses of COVID-19 vaccine by October 30, 2021.

As always, an employee who is not able to obtain a COVID-19 vaccine for a reason related to a protected ground set out in the Human Rights and Anti-Harassment/ Discrimination Policy can request accommodation using the process set out in the City’s Accommodation Policy.

Mandatory vaccine disclosure QR codeProof of vaccination status can be securely uploaded by scanning the QR code or by visiting s.cotsurvey.chkmkt.com/psdocumentation. Staff can also submit documentation to emsdocumentation@toronto.ca (please ensure that both your full name and employee number are included in the email). You can access a copy of your vaccination record at covid19.ontariohealth.ca/.
